Nonprofit Jake’s Journey Home reached a major milestone Thursday afternoon as community leaders, volunteers and supporters gathered for the official ribbon cutting and grand opening of its new Community Assist Center, a permanent hub designed to expand and strengthen services for individuals facing housing instability and complex life challenges across the region.
The new center, located at 5936 Main Ave. in Orangevale, marks a significant step forward for the grassroots organization, which has spent years providing outreach through its Mobile Outreach Unit throughout Folsom, Orangevale, Fair Oaks, Citrus Heights and surrounding Sacramento County communities. While that mobile outreach will continue, the addition of a fixed location allows the organization to offer more consistent, in-depth and coordinated support.

Jake’s Journey Home is excited to announce the grand opening of our new Community Assist Center—a dedicated space designed to expand our support to Folsom, Orangevale, Fair Oaks, Citrus Heights and surrounding areas in Sacramento County, strengthen our impact, and help even more individuals move toward stability, dignity, and a successful future.
For years, our Mobile Outreach Unit has met people wherever they are. That service will always continue.
But now, with a central location available, we can help individuals stay connected, continue their progress, and access resources more consistently.
The System’s Missing Link
Jake’s Journey Home has become the system’s missing link—reaching people who might not otherwise seek help, or who simply do not have the ability to navigate complex systems on their own. We fill the gaps where traditional services fall short, ensuring no one gets lost, overlooked, or left behind.
What’s New at the Community Assist Center?
All of our core services remain the same, but the new space allows us to provide:
• Extensive, personalized problem-solving for individuals facing layered challenges
• A consistent, welcoming place for ongoing follow-up and support
• Seamless communication with the legal system to help individuals get their lives and records cleaned up
• Greater stability and structure as people work toward sustainable, successful futures
